TUMEUR ODONTOGÈNE ADÉNOMATOÏDE : À PROPOS D’UN CAS ET REVUE DE LA LITTÉRATURE

Résumé
La tumeur odontogénique adénomatoïde est une tumeur épithéliale odontogène bénigne. C’est un néoplasme rare représentant 3% de toutes les tumeurs odontogènes. Notre objectif est de rapporter un cas de tumeur odontogénique adénomatoïde et de décrire ses caractéristiques.
Il s’agissait d’un homme de 45 ans, ayant consulté en juin 2022 pour une tuméfaction maxillaire droite évoluant depuis plusieurs mois. L’examen clinique a objectivé une tuméfaction maxillaire droite dure, indolore, bien limitée. L’examen physique mettait en évidence une masse endo-buccale en regard de la tuméfaction. La dentition du maxillaire droite était désordonnée avec des dents mobiles. Les aires ganglionnaires cervicales étaient libres. La radiographie montrait une lésion intra-osseuse ne contenant pas de dent incluse. Le patient bénéficiait d’une maxillectomie partielle droite. À la macroscopie, la pièce opératoire était un fragment osseux non orienté mesurant 7x4x3 cm présentant une tumeur blanchâtre, bien limitée, ferme de 3,5x3x2cm sans dent incluse. Les tranches présentaient des foyers microkystiques et hémorragiques. À la microscopie, la tumeur correspondait à une prolifération tumorale composée par des cellules épithéliales fusiformes de type améloblastique. Elles formaient des nodules avec parfois des structures tubuliformes dans un fond fibreux peu abondant. Il existait par endroits des foyers de calcification. Le diagnostic retenu était une tumeur odontogène adénomatoïde extrafolliculaire, d’exérèse complète.
La tumeur odontogène adénomatoïde est une tumeur bénigne relativement rare. Malgré ses caractéristiques spécifiques, un examen histologique minutieux est nécessaire. Le diagnostic est anatomopathologique. Il conditionne la prise en charge thérapeutique.Abstract

Adenomatoid odontogenic tumor is a benign odontogenic epithelial tumor. It is a rare tumor lesion representing 3% of all odontogenic tumors. Our objective is to report a case of adenomatoid odontogenic tumor and to describe its characteristics.
The patient was a 45-year-old man, with no previous history, who consulted in June 2022 for right maxillary swelling that had been evolving for several months. Swelling of the right maxilla that had been evolving for several months. The clinical examination showed a hard, painless, well-limited right maxillary swelling. The examination of the oral cavity revealed an endo-buccal mass opposite the swelling. The dentition of the right maxilla was disordered with loose teeth. The cervical lymph nodes were free. The radiograph showed an intraosseous lesion with no impacted teeth. The patient underwent partial. Macroscopically, the operative specimen was a non-oriented bone fragment measuring 7x4x3cm with a whitish, well-limited, firm tumor measuring 3.5x3x2 cm with no associated impacted teeth. The sections were whitish, firm, with microscystic and hemorrhagic foci. On microscopy, the tumor corresponded to a tumor proliferation. Composed of spindle-shaped epithelial cells of ameloblastic type. They formed nodules with sometimes tubuliform structures in sparse fibrous background. There was calcification in some places. The diagnosis retained was an odontogenic tumor extrafollicular adenomatoid tumor, completely removed.
Adenomatoid odontogenic tumor is a relatively rare benign tumor. Although it has specific characteristics, a careful histological examination is necessary. The diagnosis is anatomopathological. It conditions the therapeutic management.
