THÉRAPEUTIQUE D’UN ÉDENTEMENT UNITAIRE ANTÉRIEUR : CAS DE BRIDGE COLLÉ CANTILEVER

Résumé
L’édentement est un problème fréquent et la prothèse amovible est la solution la plus répandue même en cas d’édentement unitaire à Madagascar. Actuellement, l’intégration des bridges collés cantilevers dans la pratique des odonto-stomatologistes nous a incité à réaliser une étude sur ce type de prothèse dont l’objectif a été de déterminer son efficacité sur un édentement unitaire antérieur.
Une série de cas cliniques a été réalisée durant 15 mois, de juin 2021 au septembre 2022. Les patients avec édentement unitaire antérieur et dent pilier saine ou une carie superficielle ou une restauration ont été inclus. Les patients non motivés, non coopératifs pour les démarches thérapeutiques ont été exclus.
Les bridges collés cantilever ont été faciles et rapides à réaliser. En bouche, ils ont une bonne intégration parodontale, sont très esthétiques, mini-invasifs avec une absence de contact de la pontique avec l’antagoniste. Les patients les ont trouvés très satisfaisants, très confortables et de moindre coût. Aucun problème parodontal, dentaire, esthétique, fonctionnel ni de décollement ou fracture n’a été observé après 9 mois de fonction.
À l’issue de cette étude, nous avons pu démontrer que le bridge collé cantilever est une prothèse innovante de bonne qualité, mini invasive, pertinente et efficace pour un édentement unitaire antérieur. Ainsi nous suggérons de l’intégrer dans la pratique des odonto-stomatologistes à Madagascar. Une étude avec un suivi de longue durée est nécessaire pour vérifier sa longévité.Abstract

A tooth loss is a problem often encountered and the removable prosthesis is the most widespread solution even in the case of a single tooth loss in Madagascar. Currently, the integration of cantilever bonded bridges in the practice of odonto-stomatologists has prompted us to carry out a study on this type of prosthesis, the objective of which was to determine its effectiveness on anterior single tooth loss.
A serie of clinical cases was carried out over 15 months, from June 2021 to September 2022. Patients with an anterior single tooth loss with a healthy abutment tooth or superficial carie or a restoration were included. Patients who were unmotivated and uncooperatived for the therapeutic approaches were excluded.
The cantilever bonded bridges were easy and quick to realize. In the mouth, they had a good periodontal integration, were very aesthetic, minimally invasive with an absence of contact of the pontic with the antagonist. Patients have found them very satisfying, very comfortable and less expansive. No periodontal, dental, aesthetic, functional problem or detachment and fracture were observed after 9 months of function.
At the end of this study, we were able to demonstrate that the cantilever bonded bridge is an innovative prosthesis of good quality, minimally invasive, relevant and effective for anterior single tooth loss. Thus, we suggest integrating it into the practice of odonto-stomatologists in Madagascar. A study with a long-term follow-up is necessary to verify the longevity of this bridge.
