  PRISE EN CHARGE PARODONTALE DES PATIENTS ATTEINTS D’AFIBRINOGENEMIE

  Résumé :
  L’afibrinogénémie est une coagulopathie rare de transmission autosomique récessive défini par une absence congénitale du fibrinogène, protéine impliquée dans la coagulation.
  L’objectif de cette étude est d’établir des protocoles thérapeutiques de prise en charge parodontale des patients atteints de cette pathologie.
  L’étude a été menée auprès de cinq patients, âgés de 4 à 13 ans, issus de l’Association pour le Bien-être des Hémophiles à Madagascar (ABEHM) ; consultés et suivis au service d’hématologie du CHU-JRA d’Antananarivo. Ont été évalués le niveau d’hygiène orale des patients ainsi que l’état parodontal des patients afin d’établir les thérapeutiques parodontales.
  Aucun des patients n’ont bénéficié de traitements parodontaux antérieurs justifiant le taux élevé de l’indice de plaque. Ils présentaient tous une gingivorragie allant de légère à sévère nécessitant une hospitalisation.
  Une amélioration de l’hygiène bucco-dentaire contribuerait non seulement à la réduction de l’inflammation et des saignements gingivaux mais également à prévenir la survenue d’une autre atteinte parodontale ou l’aggravation de la gingivite existante qui sont source d’hémorragie. L’établissement d’un plan de traitement adapté à chacun de cas patients a été mis en place en évitant tout acte hémorragique. Dans le cas où ce dernier est nécessaire, l’intervention se déroulera en milieu hospitalier et sous transfusion de plasma frais congelé PFC.
  Cette étude a mis en évidence l’importance de la prise en charge parodontale des patients afibrinogénémiques pour traiter et prévenir le saignement gingival. Une étroite collaboration entre parodontiste, odonto-stomatologiste et hématologiste est souhaitable pour une meilleure prise en charge des patients atteints d’afibrinogénémie à Madagascar.

  Abstract :

  Afibrinogenemia is an autosoma recessive disease and a rare bleeding disorder defined by a congenital absence of fibrinogen, a protein involved in coagulation. The objective of this study is to draw up therapeutic protocols for periodontal management of afibrinogenemia malagasy patients.
  The study was conducted in five patients, aged 4-13 years, from the Association for the Wellbeeing of Malagasy Hemophiliacs (ABEHM), consulted and followed within the CHU-JRA in Antananarivo. The level of oral hygiene and the periodontal status were evaluated for diagnostic and establishment of periodontal care.
  None of the patients received previous periodontal treatment which explained the elevated rate of plaque index. They all showed gingival bleeding which has led to hospitalization for three patients. Improved oral hygiene contributes not only to the reduction of inflammation and gingival bleeding but also to prevent the occurrence of another periodontal disease or aggravation of existing gingivitis. The establishment of a treatment plan adapted to each patient would avoid source of bleeding for patient with afibrinogenemia. In cases surgery is necessary, it will take place in hospital using fresh frozen plasma FFP.
  This study highlighted the importance of taking care of periodontal health of patients with afibrinogenemia to treat and prevent gingival bleeding. Narrow collaboration between periodontist, oral surgeon and hematologist is desirable for a better management of these patients in Madagascar.
