  Résumé
  L’ostéonécrose des maxillaires est une exposition d’os nécrosé localisée dans la région maxillo-faciale persistant depuis au moins 8 semaines chez un patient traité ou ayant été traité par des médicaments antirésorptifs et anti-ostéoporotiques, tels que les biphosphonates, le dénosumab et les antiangiogéniques.
  L’objectif de ce travail est d’informer les odonto-stomatologistes Malgaches sur l’existence et la prise en charge de cette pathologie.
  Une revue de la littérature sur le sujet a été effectuée afin de comparer l’avis des auteurs.
  L’ostéonécrose débute sous forme d’ulcération de la muqueuse buccale laissant ensuite exposée l’os sous-jacent, suivi d’un séquestre osseux. Asymptomatique pendant plusieurs semaines, les manifestations cliniques vont d’une simple gêne jusqu’à des douleurs plus importantes.
  Des mobilités ou des pertes dentaires spontanées sont observées. Et en cas d’extraction, la cicatrisation ne se fait pas, la plaie reste béante pour s’élargir petit à petit.
  La prise en charge initiale consiste à évaluer l’état et la pathologie générale du patient ainsi qu’à maintenir une hygiène bucco-dentaire optimale. La décision d’entreprendre des soins et des interventions chirurgicales durant la thérapie avec ces molécules doit être discutée entre l’oncologue et l’odonto-stomatologiste. Une fois diagnostiquée, l’ostéonécrose peut être traitée par voie conservatrice ou chirurgicale.
  La prévention permet d’éviter l’ostéonécrose des maxillaires. Le patient doit être capable d’informer l’odonto-stomatologiste sur sa maladie et sur le traitement qu’il a entrepris. La relation entre oncologue et odonto-stomatologiste permet d’anticiper la survenue de complications.

  Abstract
  Stomatological treatment of osteonecrosis of the jaws. Osteonecrosis of the jaws is an exposition of necrosed bone in the maxillofacial region persisting since at least 8 weeks in patient treated or having been treated with antiresorptives and anti-osteoporosis. Among these drugs, we can cite biphosphonates, denosumab and antiangiogénics.
  The aim of this article is to inform Malagasy odonto-stomatologists about the existence and the way of managing this pathology.
  A review of littérature was done to compare the autors’opinions.
  Osteonecrosis appears initialy as a buccal mucosa leting sous-jacent bone exposed, then bone sequestre can be formed.
  Asymptomatic during many weeks, clinical manifestations may be accompagned with a simple gene until more important pain. Mobilities or loss of teeth are observed. And in case of extraction, cicatrisation does not occur, the scare stays gaping and becomes large little by little.
  The first care is to evaluate the patient’s status and general pathology of the patient and to maintain an optimal oral hygien. The decision to do cares and surgical intervention during the therapy must be discussed between oncologue and odonto-stomatologist. Once diagnosticated, osteonecrosis can be treated by conservation or surgical way.
  Prevention is the best way to avoid osteonecrosis of jaws. The patient must be able to inform the odontostomatologist about his disease an treatment. The relationship between oncologue and odonto-stomatologist are essentiel to anticipate complications.
