  LES CELLULITES CERVICO-FACIALES ODONTOGÈNES DANS LA VILLE D’EBOLOWA : À PROPOS DE 49 CAS

  Résumé
  Introduction : Les cellulites cervico-faciales odontogènes sont des infections polymicrobiennes des tissus cellulo-adipeux, dont l’évolution peut engager le pronostic vital, dans les zones dépourvues d’infrastructures sanitaires. Elles nécessitent un traitement urgent et approprié. Le but de l’étude était de décrire le profil épidémiologique, les aspects cliniques et thérapeutiques des cellulites odontogènes à Ebolowa.
  Methodologie : Il s’agissait d’une étude descriptive prospective, effectuée à Ebolowa courant 2018, qui incluait tout patient consentant, présentant une cellulite odontogène. Les données ont été collectées et analysées et traitées par le logiciel CS.pro 7.1.
  Resultats : Sur 378 consultations, 49 cas de cellulites odontogènes ont été diagnostiquées, soit une prévalence de 12,9%. Les femmes étaient prédominantes avec 59,2%. La tranche d’âge [20-30[ et les élèves étaient majoritaires avec chacun 34,7% et 63,7% de cas présentant une tumeur cervico-faciale douloureuse, évoluant depuis plusieurs jours. Comme facteurs favorisants, la mauvaise hygiène bucco-dentaire (75,5%) suivi par la prise d’AINS (71,3%) étaient les plus marquants. 87,7% de molaires mandibulaires étaient touchée, avec la carie dentaire comme la principale cause (79,6%). Le trismus était présent chez 87,7% de cas. Selon les formes évolutives, 93,9% étaient aigües circonscrites. Un traitement à base d’antibiothérapie, antalgique, antiseptique associé à l’exodontie (87,7%) et l’incision-drainage étaient initiées. 89,8% de cas ont guéris et 4,1% décédés.
  Conclusion : Les cellulites odontogéniques sont fréquentes. La mauvaise hygiène bucco-dentaire et l’automédication sont les facteurs favorisants. Elles restent une urgence car le pronostic vital du patient peut être engagé. Le traitement médico-chirurgical a été difficile à réaliser à cause du bas niveau économique.

  Abstract
  Introduction : Odontogenic cervico-facial cellulitis is a polymicrobial infection of the celluloadipose tissues, whose evolution can be life-threatening, in areas without health infrastructures. They require urgent and appropriate treatment. The aim of the study was to describe the epidemiological profile, clinical and therapeutic aspects of odontogenic cellulitis in Ebolowa.
  Methodology : This was a prospective descriptive study, carried out at Ebolowa during 2018, which included any consenting patient with odontogenic cellulitis. The data were collected, analysed and processed using CS.pro 7.1 software.
  Results : Out of 378 consultations, 49 cases of odontogenic cellulitis were diagnosed, a prevalence of 12.9%. Women were predominant with 59.2%. The age group [20-30[ and students were in the majority with each 34.7% and 63.7% of cases presenting a painful cervico-facial tumor, evolving for several days. Poor oral hygiene (75.5%) followed by NSAID use (71.3%) were the most important factors. 87.7% of mandibular molars were affected, with dental caries as the main cause (79.6%). Trismus was present in 87.7% of cases. According to the progressive forms, 93.9% were circumscribed acute. A treatment based on antibiotic therapy, analgesic, antiseptic associated with exodontics (87.7%) and incision-drainage was initiated. 89.8% of cases recovered and 4.1% died.
  Conclusion : Odontogenic cellulitis is common. Poor oral hygiene and self-medication are the contributing factors. They remain an emergency because the patient’s vital prognosis can be engaged. Medical-surgical treatment has been difficult to achieve because of the low economic level.
