  Résumé :
  Introduction : La lidocaïne est constamment disponible à Madagascar. Elle peut être utilisée en anesthésie locorégionale malgré sa courte durée d’action. Nous rapportons l’expérience du CHU de Stomatologie et de Chirurgie Maxillo-faciale d’Antananarivo dans les interventions de longue durée de la face. Nous avons utilisé lidocaïne 2% sans adrénaline additionnée de fentanyl. L’objectif est de décrire les effets du fentanyl sur la durée de l’anesthésie en per- et postopératoire. Observations : Deux patients ont été opérés sous anesthésie locorégionale du nerf trigéminal en 2011. Le premier patient a reçu 12ml de solution anesthésique (260mg de lidocaïne 2% + 50µg de fentanyl). Son

  intervention a duré 120mn. Le second patient a subi trois interventions. Une anesthésie associait 250mg de lidocaïne et 50µ g de fentanyl. Cette opération a duré 95mn. Dans les restes des cas, l’anesthésie associait 400mg de lidocaïne et 200µg ou 300µg de fentanyl. Les interventions ont duré respectivement 130 et 190mn. Une sédation est observée avec 300µ g de fentanyl. Dans tous les cas, l’échelle numérique postopératoire était inférieure à 3. Conclusion : Les doses de 100 à 200µg de fentanyl sont efficaces pour les opérations de plus de 130mn, sans sédation secondaire. Les effets systémiques apparaissent à plus de 200µg.

  Abstract :
  Introduction : Lidocain is one of the most used drugs in Madagascar. It can be used for locoregional anesthesia despite its short effect. We report our own experience for the use of this drug for long duration facial surgery. We used lidocain 2% without adrenaline with fentanyl. The aim of this study was to describe the effects of fentanyl in the anesthesia duration and for the postoperative analgesia. Observations : We describe two patients which has been operated on 2011. The first patient got 12ml of anesthesia drug (260mg of lidocain 2% and 50µg of fentanyl) ; the duration of the operation was 120min. The second patient got 3 operations, and he got 250mg of lidocain and 50µg of fentanyl ; the operation lasted 95 min. For the other operations, he got 400mg of lidocain and 200 or 300µg of fentanyl. The length of theirs operations were respectively 130 and 190 min. Sedation was obtained with 300µg of fentanyl. In all cases, the numeric postoperative scale were less than 3. Conclusion : The doses of 100 to 200µg of fentanyl are efficacy for operation which last for more than 130 min, without secondary sedation. Systemic effects appear for doses more than 200µg.
